Background:The prognostic significance of non-disabling relapses in people with relapsing-remitting multiple sclerosis (RRMS) is unclear. Objective:To determine whether early non-disabling relapses predict disability accumulation in RRMS.Methods:We redefined mild relapses in MSBase as ‘non-disabling’, and moderate or severe relapses as ‘disabling’. We used mixed-effects Cox models to compare 90-day confirmed disability accumulation events in people with exclusively non-disabling relapses within 2 years of RRMS diagnosis to those with no early relapses; and any early disabling relapses. Analyses...

Evaluating the implementation of a referral system for virtual pharmacy counselling in a province-wide nurse phone line

Aude Motulsky, Lara Gautier, Marie-Pierre Moreault, Janine Badr, Man Qing Liang, Amirav Davy, Arnaud Duhoux & Marie-Thérèse Lussier
Digital technology offers several opportunities to improve access to professional expertise in primary care, and the offer of various “virtual” services has exploded in the past few years. The aim of this study was to evaluate the implementation of a direct-to-consumer on-line pharmacy consultation service (Ask Your Pharmacist - AYP) to a universal phone consultation service led by the universal public health system in Quebec (811 Info-Santé), through a direct bridge. Semi-structured interviews were conducted...

Differential Momentary Reports of Stress and Affect Associated With Alcohol Consumption in Middle-Aged Versus Younger Adults

Hope Peterson-Sockwell, W. Jack Rejeski, Jason Fanning, Stephen W. Porges, Keri J. Heilman, Paul J. Laurienti & Lise Gauvin
Background: Stress is a motivator to consume alcohol, a well-documented relapse risk, and is known to differentially affect biological and psychological processes as people age. Objectives: Because alcohol consumption is known to acutely decrease stress and increase affect, this study examined differences in ratings of stress and affect in middle-aged versus younger adults who regularly consume alcohol. Methods: A sample of younger (n = 17) and middle-aged (n = 18) drinkers was studied during a...

Predictors of Food and Physical Activity Tracking Among Young Adults

Erin K. O’Loughlin, Maryam Marashi, Catherine M. Sabiston, Kristen M. Lucibello, Marie-Pierre Sylvestre & Jennifer L. O’Loughlin
Background:Monitoring food intake and physical activity (PA) using tracking applications may support behavior change. However, few longitudinal studies identify the characteristics of young adults who track their behavior, findings that could be useful in designing tracking-related interventions. Our objective was to identify predictors of past-year food and PA tracking among young adults. Methods:Data were available for 676 young adults participating in the ongoing longitudinal Nicotine Dependence in Teens Study. Potential predictors were measured in 2017–2020 at...

Advanced practice physiotherapy surgical triage and management of adults with spinal disorders referred to specialized spine medical care: a retrospective observational study

Simon Lafrance, François Desmeules, Maxime Charron, Lior M. Elkaim, Julio Fernandes & Carlo Santaguida
In this novel advanced practice physiotherapy (APP) model of care, advanced practice physiotherapists (APPTs) assess, triage, and manage adults with spinal disorders to alleviate the growing demands in specialized spine medical care. To describe this APP model of care, to assess change in disability 3 months after rehabilitation care and to assess surgical triage and diagnostic concordance between APPTs and spine surgeons. In this retrospective observational study, consecutive patients who completed the 3-month follow-up data...

The French-Canadian WheelCon for manual wheelchair users: paediatric adaptation and preliminary evaluation of its measurement properties

To adapt the Wheelchair Use Confidence Scale for Manual Wheelchair Users, French-Canadian Version (WheelCon-M-F) into a paediatric version (WheelCon-M-F-P) and to evaluate its validity. A three-phase process was conducted: (1) item adaptation using secondary analysis of focus group data; (2) item refinement using a think-aloud process; and (3) preliminary validation of the WheelCon-M-F-P (i.e. internal consistency, test-retest reliability, standard error of measurement, smallest real difference, ceiling and floor effects, limits of agreement, and relations with...

Feedback on clinical team performance: how does it work, in what contexts, for whom, and for what changes? A critical realist qualitative multiple case study

Joachim Rapin, Sylvie Gendron, Cédric Mabire & Carl-Ardy Dubois
Abstract Background Feedback on clinical performance aims to provide teams in health care settings with structured results about their performance in order to improve these results. Two systematic reviews that included 147 randomized studies showed unresolved variability in professional compliance with desired clinical practices. Conventional recommendations for improving feedback on clinical team performance generally appear decontextualized and, in this regard, idealized. Feedback involves a complex and varied arrangement of human and non-human entities and interrelationships....

Transnational prenatal care among migrant women from low-and-middle-income countries who gave birth in Montreal, Canada

Lisa Merry, Ye Na Kim, Marcelo L. Urquia, Julie Goulet, Sarah Fredsted Villadsen & Anita Gagnon
Abstract Objectives There is little research examining transnational prenatal care (TPC) (i.e., prenatal care in more than one country) among migrant women. Using data from the Migrant-Friendly Maternity Care (MFMC) - Montreal project, we aimed to: (1) Estimate the prevalence of TPC, including TPC-arrived during pregnancy and TPC-arrived pre-pregnancy, among recently-arrived migrant women from low- and middle-income countries (LMICs) who gave birth in Montreal, Canada; (2) Describe and compare the socio-demographic, migration and health profiles...

National standards or territorial autonomy? Public opinion and the politics of fiscal federalism for healthcare in Canada

Sophie Borwein, Olivier Jacques, Daniel Béland & André Lecours
Focusing on public opinion, this paper studies the politics of fiscal federalism by analysing support for the level and conditionality of intergovernmental transfers for healthcare. Leveraging a representative survey of the Canadian population conducted in 2022, we show that preferences for fiscal federalism reflect territorial identity and ideology. Left-wing respondents are more likely than right-wing ones to prefer higher, conditional transfers. However, Canadians with a predominant provincial identity prefer more decentralisation and therefore unconditional transfers,...

Abstract Background The majority of people with a chronic disease (e.g., diabetes, hypertension, COPD) have more than one concurrent condition and are also at higher risk for developing comorbidities in mental health, including anxiety and depression. There is an urgent need for more relevant and accurate data on digital interventions in this area to prepare for an increase demand for mental health services. The aim of this study was to conduct a meta-analysis of the...
